Fisting: Hands, Lube, Leather History, and the Injury Risk

Fisting, also called fist fucking (FF) or handballing, is brachioproctic when the hand goes into the rectum and brachiovaginal when it goes into the vagina. One or more hands enter. People do it with a partner or alone. In BDSM and leather culture it is a high-skill deep-penetration practice, not a speed contest copied from porn. The people who do it are often called fisters. Gloves, trimmed nails, the amount of lubricant, and slow practice matter more than the pace in adult video. Everyone involved is a consenting adult.

From back rooms to a named history

Where fisting came from is disputed. Some people place the visible rise in twentieth-century gay male culture. Others say the eighteenth century or earlier. Sex educator Robert Morgan Lawrence has argued the practice is thousands of years old. As a visible subculture it grew with gay liberation. In the hanky code, a red handkerchief signaled interest in fisting. By 1973, South of Market venues in San Francisco, including the Red Star Saloon, were advertising it openly. The best-known dedicated space was the Catacombs, a gay and lesbian S/M leather fisting club in San Francisco, open from 1975 to 1984, with a move partway through. Other names in that world were Handball Express and Red Hanky Express. Crisco, the shortening, was the classic lubricant. It shows up in the 1980 film Erotic Hands. Purpose-made fisting lube did not exist yet.

The AIDS crisis of the 1980s hit this culture hard. San Francisco, New York, and other cities closed bathhouses and sex clubs believed to allow unsafe sex. Most fisting venues went with them. Community voices such as Randy Shilts supported the crackdown. The gay press and the San Francisco AIDS Foundation, among others, called the closures a civil-liberties attack driven by anti-gay feeling. In the twenty-first century the culture warmed again as HIV treatment and prevention improved, internet porn spread, and apps including Recon made people easier to find. In 2017 the South of Market Leather History Alley in San Francisco put Catacombs owner Steve McEachern and educator Bert Herrman on its map.

Hand shapes, depth, and the words people use

The hand shape beginners are usually pointed toward is the silent duck, or duck-billing. Four fingers stay extended and the thumb tucks into the palm, so the hand looks like a beak and can enter before the tissue is fully relaxed. Punchfisting is further along: part or all of the fist is used before, during, or after entry. Double fisting uses both hands at once. The stretch often matters more to people than thrusting. Depth commonly stops around the wrist. Experienced players may go toward the elbow. Some scenes put a hand in with a penis or a toy in the same opening. Three hands is rare and advanced.

The vocabulary is specific. The person doing it is a fister or fist-fucker. The person receiving is a fistee. When rectal tissue pushes out like rose petals, that is a rosebud, or rectal prolapse. Bloom is the verb for producing that look. American slang dictionaries date the word fisting to no later than 1969. Dildos are often used to open the body or add sensation. Water-based and hybrid fisting lubes are sold now because classic oil-based lubricant fights with latex and with some toys.

Injury, preparation, and what the research actually says

The medical risk is real. Fisting can cut or perforate the vagina, perineum, rectum, or colon. Serious injury is documented, and so are rare deaths. Torn mucosa can raise the chance of sexually transmitted infection. Forcing air into the vagina can cause a fatal air embolism, especially in pregnancy. A 2021 study found that men who practiced anal fisting reported fecal incontinence in the previous month at about 18.1 percent, against 7.2 percent for other men who had anal sex. A 2022 report in the journal Sex Education argued that, carefully framed, the STI risk can be close to none, and that clinical writing sometimes inflates the danger. The injuries are still real.

Experienced educators put preparation ahead of bravado. Nails short and filed smooth. Jewelry off. Nitrile gloves give a barrier and a better grip. Lubricant has to be plentiful, thick, and added again and again. The silent duck is popular because driving a whole fist into tissue that is not ready is how lacerations happen. Communication stays continuous. Stretch intensity and pain that means tissue damage are different things. Afterward, watch for bleeding, fever, or severe pain. In pregnancy, if you do not have the medical knowledge, treat the air-embolism risk as a hard limit. Depth play and punchfisting are advanced specialties. They are not a beginner’s badge.

Pictures, clubs, and the law

The image has a long trail. Robert Mapplethorpe photographed Double Fist Fuck (1977) and Helmut and Brooks, NYC, 1978, among other fist pictures. Tom of Finland’s leather drawings, Kent Monkman’s Hanky Panky (2020), and Ron Athey’s 1999 performance Solar Anus all touch it. In music there is Rod McKuen’s Slide… Easy In and Tool’s Stinkfist. Caligula, Cruising, and South Park have referenced or joked about it. Gay porn titles such as Drive and The Other Side of Aspen recorded the same culture.

Historic gathering places include the Mineshaft in New York (1976–1985), the Caldron in San Francisco, the Crisco Club in Florence, and La Fistinière in France (2007–2018). Since 2011, Jiz Lee and others have pushed 21 October as International Fisting Day, a celebration and an anti-censorship date. In the United Kingdom, the Crown Prosecution Service did not clearly stop prosecuting fisting images of consenting adults until 2019. Before that, “extreme porn” fights included the 1998 seizure of Mapplethorpe holdings and the 2012 acquittal in R v Peacock. In the United States, the 2001 Cambria List named fisting among acts producers feared under obscenity law. By 2019 the industry widely treated that list as obsolete. In this culture, fisting is leather history and craft, and it has also been a point where law and sex collided.
